Reservation and Release Form

The areas fished can have some wave or “chop”.  Therefore, I strongly recommend that if you have any tendency toward motion sickness (or are not sure if you have any tendency), consider taking appropriate precautions such as motion sickness prevention medication.  Locations fished may not allow a person to be returned to the dock for several hours which could make an otherwise very pleasant day, unpleasant.

 

The physical nature of the waters, weather, contents inside the boat (including sharp objects) and activities by people around the boat and water can be unpredictable. Precautions are taken to prevent unexpected occurrences however there is no guarantee that events will not occur.  By making this reservation, you understand and are accepting any risk from such occurrences.  You also understand that you will be required to obtain the necessary licenses from the local jurisdiction (state) where you will be fishing (these can often be obtained the day before, or day of, the first trip).  Any photographs taken by, on behalf of or provided to Chinook Guide Service may be used for the promotion of Chinook Guide Service and you are authorizing any photos taken to be used for those purposes, without compensation.

 

Fishing trips often begin early morning and last a minimum of 8 hours.  Age considerations need to be made.  Generally anyone 10 years or older may be able to fish in the boat , however, each person needs to consider all factors including an individuals capacities and where the fishing may be occurring.  For the primarily used boat, the width (beam) is 98” and inside floor height is 26”.  Waters and fishing situations can produce unpredictable occurrences.

 

All fishing gear is provided by Chinook Guide Service however in addition to other considerations, each person will need to provide the following:

 

 

 

 

 

 

CANCELLATION POLICY:  If I cancel your trip for any reason up to the day and time the trip is to begin, you will receive a full refund of your deposit (allow 2-4 weeks).  Once a trip begins, the full fee applies and is non-refundable.   If you cancel your trip:  Longer than 31 days in advance (if multiple consecutive days are involved, then this would apply to the first day of those consecutive days):  A full refund will be given of your deposit (Allow 2-4 weeks).  Within 30 days (if multiple consecutive days are involved, then this would apply to the first day of those consecutive days): Your deposit is forfeited.  However, if within that 30 day time frame, you provide written notice of cancellation and I am able to book your cancelled date with another customer, I will apply your deposit toward another trip of equal value provided that trip is booked for the same time frame and year (i.e., spring, summer/fall, etc).

Please call me (503-407-3168) to confirm availability.  Read and complete the needed information, then each person mail one original copy together with your deposit (make one copy for your records).  No reservation is complete without this form and your deposit!

 

While the pictures of fish and boats are true, nothing about this guarantees that you or your party will catch fish.  All information provided by Chinook Guide Service and Dave Steele are represented as true as possible to the original information.  A deposit of one-half the per person daily charge is required and a tentative reservation made by phone is good for seven (7) days.  No reservation is complete until the deposit is received by Chinook Guide Service (a check that doesn’t “clear” a bank is considered as a deposit not received).  No visibly intoxicated person will be allowed to fish on the boat and any deposit WILL BE forfeited!
